High School Documentation for Student Access (formerly Disability) Services

We encourage any student who had a 504 plan, an Individualized Educational Plan (IEP) or who participated in special education services to consider registering with the SAS. Additionally, we encourage anyone with a documented disability who did not participate in special education services to contact us.

Even if you don’t believe you will need academic adjustments, we recommend connecting with SAS as an additional resource.

Accommodation Process

  • Once registered, students choose which instructors receive their accommodation letters.
  • New accommodation letters must be requested each semester.
  • Accommodation letters only list approved academic adjustments and do not disclose the student’s diagnosis.

Confidentiality and Documentation

  • All submitted documentation is kept confidential and accessible only to SAS staff.
  • This documentation is not part of a student’s academic record and does not appear on transcripts, diplomas, or certificates.
  • SAS cannot release documentation without the student’s written permission.

We recommend that students contact Student Access Services (SAS) as soon as they apply to HACC. However, students may reach out at any time during their academic journey.

Processing Timeline

  • Once you submit your information, it may take two or more weeks to review your request and schedule an appointment.
  • Additional documentation may be required from students, medical providers, or faculty to assess requests.

Avoiding Delays

  • Start the registration process as early as possible.
  • Submit all required supporting documentation promptly.
  • Respond quickly to requests for additional information or meetings.

Delays in any of these steps may impact when accommodations can be provided. To ensure timely support, we encourage students to begin the process early.

Will My Accommodations Transfer If I Move to Another College?

  • Accommodations do not automatically transfer to another school.
  • Each institution has its own process, and students must register separately with the disability support office at their new college or university.

How to Get Started

To begin the process, please complete our referral form. Students may self-refer or be referred by a faculty or staff member for ADA or Title IX Pregnancy and Parenting support.
