HACC Transcript Request Form (pdf)

Students may request a transcript of their permanent academic record either by appearing in person at the Registration Office/Welcome Center at any HACC campus, by submitting a request through HACCWeb, or by written request to the Records Office, HACC, Central Pennsylvania's Community College, One HACC Drive, Harrisburg, PA 17110.

Written requests must include:

• Your name
• Student identification number (HACCid) or last four digits of your SSN
• Home address
• Daytime telephone number
• Address where the transcript is to be sent
• Your signature
• $3 payment for each official transcript requested
• Date of birth

Unofficial transcripts may be requested at no charge.

You may print the pdf form listed above. Either mail the completed form to the HACC, Central Pennsylvania's Community College, Records Office, One HACC Drive, Harrisburg, Pa 17110 or submit the form to the Registration Office/Welcome Center at the campus you attend.
