Major and/or Advisor Change

You may use the form listed below to change your advisor or major. Forms are also available at the Welcome Center of the campus you attend. Due to state requirements, your signature is required when you change your major. Your advisor's signature is also required. Program change requests after the 3rd week of the term become effective in the next term. Specific deadline dates can be found under Registration Dates/Deadlines.
