Documentation Required for Medical or Physical Disability

Students are responsible for submitting a letter, typed on official letterhead, from a qualified physician or appropriate medical practioner containing:

• the evaluator’s name, title, professional credentials and assessment date
• the specific disability
• the limits the disability imposes, especially the impact of the disability on a major life activity in an educational setting
• recommended accommodations/academic adjustments
• duration of the disabling condition
